Renewal Fee Draft, Prescribed application form & Form L, Original Affidavit,
Attested copy of Matric Certificate, Attested copy of last Fee paid Receipt, photocopy
duly attested of the original Registration Certificate of the Council, Attested
copy of Diploma OR Degree of Pharmacy, Attested copy of Ration Card and Attested
copy of at least two CME certificate.
